Writing a check to Dr. Arnold's office?

I am trying to send off my packet to Dr. Arnold's office and realized this morning that I didn't ask them who to make out the check to. I was hoping someone here might know off the top of their head so that I didn't have to wait for them to return the call.

In my call to them only yesterday, I was told that the entire staff comprises just the doctor, the nurse, and the assistant (Tammy, with whom I spoke). They'll take cash, a check, or a money order. Their correct coordinates (as of 11/10/08) are:

phone 512-444-3131. I forgot to ask whether Dr. Arnold is a man or a woman.

06-04-2007 = TSC stamps postal return-receipt for I-129f.

06-11-2007 = NOA1 date (unknown to me).

07-20-2007 = Phoned Immigration Officer; got WAC#; where's NOA1?

09-25-2007 = Touch (first-ever).

09-28-2007 = NOA1, 23 days after their 45-day promise to send it (grrrr).

10-20 & 11-14-2007 = Phoned ImmOffs; "still pending."

12-11-2007 = 180 days; file is "between workstations, may be early Jan."; touches 12/11 & 12/12.

12-18-2007 = Call; file is with Division 9 ofcr. (bckgrnd check); e-prompt to shake it; touch.

12-19-2007 = NOA2 by e-mail & web, dated 12-18-07 (187 days; 201 per VJ); in mail 12/24/07.

01-09-2008 = File from USCIS to NVC, 1-4-08; NVC creates file, 1/15/08; to consulate 1/16/08.

01-23-2008 = Consulate gets file; outdated Packet 4 mailed to fiancee 1/27/08; rec'd 3/3/08.

04-29-2008 = Fiancee's 4-min. consular interview, 8:30 a.m.; much evidence brought but not allowed to be presented (consul: "More proof! Second interview! Bring your fiance!").

05-05-2008 = Infuriating $12 call to non-English-speaking consulate appointment-setter.

05-06-2008 = Better $12 call to English-speaker; "joint" interview date 6/30/08 (my selection).

06-30-2008 = Stokes Interrogations w/Ecuadorian (not USC); "wait 2 weeks; we'll mail her."

07-2008 = Daily calls to DOS: "currently processing"; 8/05 = Phoned consulate, got Section Chief; wrote him.

08-07-08 = E-mail from consulate, promising to issue visa "as soon as we get her passport" (on 8/12, per DHL).

08-27-08 = Phoned consulate (they "couldn't find" our file); visa DHL'd 8/28; in hand 9/1; through POE on 10/9 with NO hassles(!).

Dr. Arnold is a woman....

I take that back.... Looked on the internet and it said the doc. was a male... Someone had told me that the Doc was a she... Oh well!

I haven't called Dr. Arnold's yet about having him transfer my husband's vaccine record onto the 693. For those of you who have actually used Dr. Arnold, is it completely neccesary to first call him. Or could I just send him an envelope in the mail to his address including the 693 form, a check for 10 dollars, and the vaccination record of my husband.

I'm trying to get things done ASAP, because we are already kinda behind..

Please let me know if any of you have advice on this.

It is best to call the doctors office. The secretary will let you know which parts of the forms to fill out and sign. To have a vaccination record transcribed....it is more than $10.00 If I am not mistaken, I beleive we wrote a check for $20.00. It may have even been $25.00 I can't remember/. There were other things that the secretary told us to do/send, but I do not recall. After I made the inital call to the office and left a message, we received a call back the same day.

Dr. Arnold's fee is $20 -- cash, check, or money order. I heard from them on Monday, and they indeed called me back the same day. You really do need to call their office, because the necessary vaccinations depend on a person's age.

You will need:

Two copies of the FIRST PAGE ONLY of the I-693 form, printed from the uscis.gov website. Put your original signature on each of the two copies and FILL NOTHING ELSE OUT!

A record or letter on a doctor's official stationery or official form, to show the person's vaccination history. Your own handwritten notes are not enough; it must be from a doctor.

Provide a translation of the above, if it's not in English.

On a separate page, provide the immigrant's: last name, given name, middle name; address; phone number; alternate phone if any; sex, date of birth, country & city of birth, Social Security Number if any, and the $20 as stated above. The address is in my earlier message in this thread.

I agree with the person who said call first...the lady I talked to on the phone verified that we had all of the vaccinations necessary. If something had been missing we could have gotten it taken care of before sending everything on! :)

I had thought it was a woman, too until I looked it up on-line.
