Documentation for ds-230 to NVC for Lima, Peru

Hello, We are in the process of finishing the I-130 with the NVC. I have a question does anybody now what documentation to send to the nvc along with the ds-230? Does somebody that has gone through the process have a ds-230 coversheet that I can look at? Any help will be greatly appreciated. Thanks.

Have a look here: http://travel.state.gov/visa/immigrants/info/info_3176.html

That is the link they send to you for your DS-230. Select your consulate from the drop down list, it will then take you to a page that tells you all the civil documents you need for your consulate. For information on the affidavit of support you can start here: http://travel.state.gov/visa/immigrants/info/info_1335.html

The coversheet looks like this:
