Timeline

Can anyone tell me where I can download the most up to date medical forms for packet three?

DOS no longer has them posted for downloading. In addition the ones that were posted were out of date.

If you are referring to a OMB dates, you can continue using the forms, it is a date that Office of Management and Budget specifies that suggested changes and auditing by the various departments needs to be completed by, in this case Department of State. OMB date is an internal audit date, not an expiration of the form.

The various consulates use and mail out forms that are much older than that.

For some reason visapro.com does not show a listing for any of the medical forms (ds2053, ds3024, ds3025 or ds3026). I also did a search on the website with negative results.The DOS website says these forms are unavailable. Any other suggestions??

These forms are not filled out by you or the beneficiary, they are filled out by medical personnel, so are provided in packet 4 to take to the medical before attending interview.

I receive the Medical History form in packet 3 and was instructed to complete and sign it. What the Doctor does is had his remarks a the bottom and send it back to the embassy. Packet 4 had only the appointmrnt letter for the interview.

05/01/07: NOA1 Date.....no hard copy

08/06/07: Fiance called USCIS for info aboout NOA1 hard copy

08/07/07: Touch

08/08/07: Touch

08/09/07: Touch

08/10/07: Touch

08/11/07: NOA1 hard copy

08/21/07: Touch due to phone call was told that our case has been assigned

08/27/07: Touch

08/28/07: APPROVED!!!!!!!!!!!!!!!!!!!!! ( Received e-mail that our case has been approved on the 27th)

09/18/07: Petition left CSC

09/24/07: NVC received Petition

09/26/07: Petition left NVC

10/02/07: Kingston Embassy received Petition

10/04/07: sent off DS-230 part 1 (sent via courier service)

10/09/07: Packet 3 received

10/09/07: Drop off DS-230 part 1 (in person)

12/03/07: Medical

12/18/07: INTERVIEW DATE!!!!!!!!!!

01/04/08: VISA IN HAND!!!!!!!!

01/11/08: Arrived in the USA

03/23/08: WEDDING DATE

09/19/08: AOS approved

09/25/08: GC in hand

Timeline

She should receive the medical forms in her packet 3, she must then take all of these forms to the hospital when she has her medical done for the doctors to fill out (suggest you wait till she receives her packet 4 with her interview appointment date, she should plan on going to the hospital a couple weeks before her interview). Another option is to wait till her case is received in Bangkok, then email the IV section and request that they email you the current forms.

02/20/2005 - Met first time in Bangkok, Thailand

01/19/2006 - "Unofficial" Wedding Ceremony in Surin, Thailand

03/19/2007 - Filed K-1 Visa petition at Vermont Service Center

08/30/2007 - Received K-1 Visa from US Embassy Bangkok

10/05/2007 - Arrived POE at Washington DC

10/09/2007 - "Official" Wedding Ceremony in Rockville, Maryland

10/23/2007 - Apply for SSN

10/25/2007 - Sent AOS/EAD/AP petitions to Chicago Lockbox

10/29/2007 - Received SSN

11/02/2007 - Received NOA for AOS/EAD/AP

11/27/2007 - AOS/EAD Biometrics - USCIS Glenmont, Maryland

12/19/2007 - AOS transfered to CSC

12/27/2007 - EAD/AP approved

01/03/2008 - Received AP Documents

01/05/2008 - Received EAD Card
