There is no medical to be done for the AOS...it's done before the K1 interview. You will need to make sure you fiance brings their vaccination records (supplement 693A) or have the vaccines transcribed by a civil surgeon. Fee varies.

It cost me $50 for a Civil Surgeon to look over my vaccination record and sign the I-693A.

The best you can do is use the USCIS Civil Surgeon locator on their site and contact the ones in your area to ask about their fees.
