Hello everybody:

My mother in-law's IV was approved, she received her passport with the visa in it, however, no envelope was received (the one you are not supposed to open and give to the officer at POE).

When she went for medical examination, all the results were sent to the embassy digitally, she did not have to go to the interview with any papers.

Is this normal now? she wont have to carry an envelope at POE? there is a notation on her visa that reads: "IV DOCS IN CCD" what does that mean?

"Immigrant Visa Documents in Consular Consolidated Database".  It means the visa case files will be sent electronically, so no need for sealed envelope for the visa holder to carry and hand over to CBP at POE.
