Form I-693 (medical)

My spouse and I are currently working in these forms: I-130, I-131, I-485, I-765.
 

Do I need to submit form I-693 (medical records) with all the other forms as well? If so, can someone explain how that works since that document is mostly completed by a doctor .

You will need to make an appointment with an approved civil surgeon for your medical exam.   Have you done that?

I haven’t done that. I believe my question goes back to is do I have to submit this form with all the other forms as well? And how do I contact a doctor in that case?
