What do I need to bring for medical exam for visa?

The doctor will need the following items to complete the medical exam forms: Your visa interview appointment letter, your passport, your national identification card, and a copy of your immunization records. These requirements apply to all members of your family who plan to accompany you to the United States.Oct 10, 2016
