I'm filling my AOS (K1) and I haven't gotten a SSN yet. I want to get it with form I-485 so I'm checking yes on Part 1 item 15-17. (If you want SSA to issue a SSC). Do I need to send the form SS-5 along with I-485 and I-765, or I just need to check yes and that's it?

No need to send form SS-5.  Just fill out the SSN sections on forms I-485 and I-765.

If you still have more than 2 weeks on your I-94, you may also apply for SSN in person at an SSA office, if you haven't done so already.
