Hi all,

I don't know what to input my name for my i485.

In my passport:

- Last name:  Nguyen

- First name: Thi Thai Ha

When I applied for I-129F, I put down my name as below so all my documents (NOA 1, NOA 2 receipts) are the same:

- Last name: Nguyen

- First name: Ha

- Middle name: Thi Thai

But in I94 form, my name was the same as the passport. So what should I put down as my name in I485?

TIA!!

This is correct but look carefully at the visa when it is issued and later when you get the green card.  My wife and daughters visas were issued correctly but somehow they mixed up the order of the names on my wife's green card.  Daughter's was issued correctly.  I'm a bit surprised that this was an issue as I would have assumed that all of the information on the visa would have been computer transfered directly to the green card when we arrived in US.  It was not.  Going to an appointment next week at USICS to get it fixed.

Yeah, this is such a headache. Thanks for the advice, I'll keep an eye on the papers.

My visa was issued as the same as in passport so I was very confused. But I'm going to put as suggested by @Klipsch17310. My other friends were also recommending me to do so.
