Beneficiary here, UK based. 
 

my name was changed as an infant to my fathers surname as it was my mothers previously. I cannot locate the legal document and fear it is lost - I have obtained my birth certificate which states my former name of course. 
 

can you advise on how one would obtain a document dating back to 1993 when they aren’t even required to be submitted anywhere...?

 

(In UK, you can just print your own from the internet and complete and have witnessed and then begin to use and use as evidence for changing passport etc.)

I think your best bet is to try to obtain a copy of the court order for the name change, from the court where it occurred.

I actually just re-read the but in the accompanying guide to the form and it says ‘if either of you use a name other than the one shown on submitted evidence then you must provide evidence of legal name change’ etc. 
 

I don’t currently and haven’t for a long time used previous name so I suppose I won’t need to submit the document actually. Do you also interpret that the same? Since I don’t use it.

If your birth certificate is not the same as your current legal name, you will need to provide the court order.

Statutory Declaration/Change of Name Deed Poll (if applicable)

If you use a different name from the one shown on the birth certificate, you must produce a document explaining the use of such a name. Evidence includes, but is not limited to baptismal certificate/record, a deed poll and/or school records showing early use of name;

Ah of course - thank you. So just need it by the time the interview rolls round.
