What to Put in E-Passport Renewal Application Form

About to renew my e-passport tomorrow and want to keep it in my maiden name because I've been separated for almost a year and will divorce soon. GC holder since 2015, to expire 2028.  I've read in an older post that I do not need to file ROM since I can simply renew my passport in my maiden name.  From what I've gathered so far, I should put my maiden name again on the form (not follow GC married name); I should put SINGLE under the No. 7 "Civil Status" question of the form and choose Status "Separated" even if I'm still married, as we haven't filed for the divorce yet.  TIA to those who would help.🙂 

 

I really need an answer to the following please,  thank you!!

1. For question # 13 "Applicant's Spouse's Name" -- What do I have to put? N/A? (Is that legitimate  and won't cause them to question me?  Like I said we will definitely do the divorce and been separated almost a year but still technically married on paper.)

2. What other questions should I anticipate from the consulate staff?

3.  In the self-addressed envelope, I should write my name and address under the "To" section and not the "From section right"?

Hi! I recently renewed my passport so this might help:

 

I’m not sure what you can put for your question #1 as I’m not knowledgeable about the technicalities of it. 
 

For question #2, they did not ask me any questions. They just ask for the required documents and that’s it (I went to the consulate in Chicago). 
 

For question #3, yes you will just write your name and complete address under the “To” section.
