Daft I'm sure.... question... Do I need to even fill out the I-693, if I have a Valid Medicial and proof of vacines?

I looked at the form and it is mosrly for a doctor....

 

If I dont need to fill it out - Do I just submit the paperwork I was given at the medical?

 

I am on a K1 Visa - now in States

 It depends on if the vaccination page from London said you completed requirements for AOS. Yes?

 

AND the form is for a civil surgeon to fill out with you only doing a small part. Read the instructions that go with the form if your vaccinations were not signed off in London. https://www.uscis.gov/i-693
