Need help about B2 visa refusal after having lost prior issued but unused visa.

Wrong.  It is based primarily on the info provided online in the DS-160.  They are not required to look at any documents brought to a B visa interview.

Her application will be evaluated based on her current circumstances, not on her circumstances during a past application.

 

OP, you don't seem to understand that information about visa applications and use is stored and shared electronically across agencies.
