NBI Clearance With No Thumbprint & Signature

Hello Everyone,

 

My wife currently lives in Japan and have successfully obtained a Philippine NBI Clearance without having to be physically present. However, the NBI Clerance she received has no thumbprint and signature.

 

Are we supposed to submit the NBI Clearance as is or is she supposed to affix her thumbprint and signature on it?

 

We appreciate the help. Thanks in advance.
