The evidence should be reasonably recent. A few weeks to few months is likely fine.

10/24/18: Green card produced notice, 10/25/18: Formal approval, 10/31/18: Green card received

He just need to sign it? and its should be notarize?

Yes, signature only. It is signed under threat of perjury. No notarization needed.
