Timeline

I have my green card interview scheduled in June. I am AOS applicant through K-1. While completing my medical in my home country, I chose to get the vaccinations done & received a DS-3025 along with the sealed medical which I submitted at POE.

I did not fill out the I-693 form while submitting the AOS package & attached the DS-3025 form as proof that I got my medical & vaccination done. I did not receive any RFE.

I read on the forum that you need 2 MMR shots. However, on my DS-3025 form I have one date on the MMR portion & nothing written in the blanket waiver part.

The second part of form has Vaccination documentation as the heading & the box "K visa applicant voluntarily completed vaccination requirements" ticked.

The copies of DS-3025 that I saw on the forum looked a bit different than the one I have, they have the option US Vaccination COMPLETE option which I don't see on my form.

 

I was wondering if it is sufficient to take this form to my interview? Do I need to get any more vaccinations done & fill out form i-693 by visiting a Civil surgeon?

It is directly related to your topic. You are asking about the I-693. I made a topic about the I-693 for Adjustment of Status and I received helpful responses. You must have not read it thoroughly. My DS-3025 says the same thing as yours. I think there was a response on my topic about that, too.

My husband's DS-3025 is the same, it shows one date for the MMR vaccine, and nothing in the waiver column.  "K visa applicant voluntarily completed vaccination requirements" is checked.  We just had our green card interview last week and we were approved.

Oh okay. I apologize. I read the title & thought that the expired i-693 is not applicable to my case.

I am unable to open the image attached on your post of the Form DS-3025, it shows image broken.

 

As you replied again, I will be taking the DS-3025 with me & if they request for another vaccination, only then I will get it. Thank you for your response :)

I am not sure why that image was broken, maybe the person who posted it deleted it? It was somebody else who posted it.
