Is Form I30A needed

Hey everyone.

I was checking over my packet to make sure I had everything and was ready to send it in until I went online to make sure I had all of the documents and came across form I30A. Is that form required to send in with the I30 or is that optional?

you can fill it for your spouse. I filed it out for my spouse
