form number ? is that the receipt number right ? the one starts with some letters in the beginning ?

As far as I know, you don't need all of those documents.. you can of course bring them if it makes you feel more secure but expired conditional green card, 18-month extension letter, and current passp

I think so, it's been a while since I went and I don't feel like getting out of bed and look at the extension letter right now. But there is a number on there (might be the case number) that works.
