Additional Information I-134 Form

Guys!

We forgot to sign and date the last page of our 134 Form, where the additional info is. The info is about his second job. The form itself is sign and dated, its only the last page where its missing.

How serious is it? What do you suggest?

Directions from USCIS say this "If you need extra space to complete any item within this affidavit, use the space provided in Part 7. Additional Information or attach a separate sheet of paper; type or print your name and Alien Registration Number (A-Number) (if any) at the top of each sheet; indicate the Page Number, Part Number, and Item Number to which your answer refers; and sign and date each sheet."

 

My assumption was that if you attach your own sheet of paper to include additional information, it must be signed and dated but if you use the space for additional information that is part of the form then you only need to fill out the blanks provided. 

 

(Note: I did not sign the "additional information" page on our original I-129f petition and it caused no issue. I also didn't sign this page on the I-134, but our interview is later today).
