I have questions about filling this form. On page 4 item 26a , do i have to also include my fiancee daughters here if they are not going to travel with her?

Also the item that says Date of filling , that is the date USCIS receive my case?

How do I answer question 38 in the I-134 (Affidavit of Support) form?

I find it very strange, since there must be information about some "specific contributions". 

What are they? Or is it better to just answer "I do not intend.."?

Obviously, there will be financial support but this form is a bit confusing.
