Doctor’s medical reports

Hunted and can’t find the answer so at the risk of incurring wrath here am I right in assuming the following needed to be reported on/not needed patient summary is enough.

needed:

asthma

 Left knee surgery

episode of depression 

 

 

Not needed:

gynae issues investigated by hospital 

Right kneecap dislocation 

ibs/lactose intolerance investigated by hospital 

tonsils removed 

childbirth 

 

Or do I need all but childbirth reporting on?

I can only speak for issues I am familiar with, of course. Certainly with the depression you need a doctor's note/report. Now, my medical/patient summary noted asthma, but the last time I ever used an inhaler or had a complaint relating to it, I was seven years old (I'm 32 now). When I mentioned this at the medical exam, they didn't bother asking any more questions. So perhaps it will depend on how long it's been an issue for with you.

 

As for the other things, I will defer to those more knowledgeable than I.

just put down everything, i off the cuff mentioned i had laser eye surgery and the dr even wrote that down, and kinda shook his head like "why didn't you put this on the questionnaire", but then told me it was fine. 

at your medical you're going to go through everything anyway and they will add notes, the most important thing to understand is that you have reports for anything "significant" listed on your summary.
