Confused about I-693

Hi everyone.

 

I'll be entering USA from Canada with the sealed envelope in early October (yay!) so I'm busy compiling all the necessary AOS documents. I just had a question about the I-693. I'm aware that if I file I-485 within 1 year of the oversea medical exam then I don't need to have another medical examination. However, I'm just a bit confused about this part: 

 

Even if a new medical examination is not required, you must still show proof that you complied with the vaccination requirements. If the vaccination record (DS 3025) was not properly completed and included as part of the original overseas medical examination report, you will have to have the Part 9. Vaccination Record completed by a designated civil surgeon. In this case, you must submit Parts 1., 2., 3., 4., 6., and 9. of Form I-693.

 

What kind of proof of vaccination requirement are they looking for? Would this be a letter from my family doctor saying that I received the required vaccinations? How would I be able to know if my vaccination record was properly completed in my medical examination report? Lastly, if my vaccination record was completed and nothing was wrong with it, would I still need to make an appointment with the civil surgeon and send the completed I-693?

Timeline

The documentation of the medical and vaccinations would be in the sealed packet.  You do not provide them again when filing to adjust status.
