Hi. There seems to be a great deal of confusion surrounding exactly how the sponsor/petitioner should prove residency / domicile requirements. I read a story that a couple was refused because the ENVELOPE that contained an electric / utility bill was not included.  Seriously? Thank you for any and all advice.

Never heard of such a thing.  Domicile is where you reside.  For petitioners living in Canada, the US consulate in Montreal greatly prefers the USC to be living in the USA by interview.  This means a job that's already been started, a place to live, children registered in school, day care, and all the things that go with living at a location.  A variety of things can be included.  Most countries are okay with intent to reestablish domicile, but we share the longest border in the world.  We have generally very little issue crossing said border.  The DOS does not care that it can be financially hard on families to have two households. What they care about is the ability of the household to function in the USA without the beneficiary becoming a public charge.  

Really if you choose to try the intent to reestablish route, understand you could be making it harder. The consulate is VERY slow at making post interview decisions.  Months go by and some have had to redo medicals or activate visas within a day or two. Pick your battles.
