My birth certificate name is Addy, but Addylin is what I used ever since including all my documents. I already submitted an Nbi clearance with Addylin as may “main name” and AKA name is Addy. But few days after, Usem called me and said that What I submitted was incorrect. Do you think it’s the other way around? Should I use Addy as my main name, and Addylin as my AKA? (Usem instruction is : NBI clearance with AKA for Addy xxxx xxxx as per Birth Certificate )

We are saying enter the name EXACTLY as in the 221g email from the embassy ...  you keep changing it

My fiancee's name is Maria Ramosa.  The daughter's birth certificate had it listed as Ma. Ramosa.  The 221g asked for an AKA for "MA RAMOSA" (without the period)...that is what we sent.  Exactly what they asked for.  We haven't had a response yet but we included the 221g email and did exactly what they asked for.
