EXAMPLE: if my full name in my passport is ( AHMED MOHAMED MAHMOUD MOUSTAFA ) HOW i write it as first and last and midlle format ??????

Last Name: Moustafa

Middle Name: Mohamed Mahmoud

It is a good format. Make sure, you stay consistent with the format in all applications, forms.

Sorry I saw this so late. For my husband we wrote XXXXXXX XXXXXXXX "M" (for Mohammed) XXXXXXXX

First, middle, first inital of Mohammed and lat name.
