Thai official misspelled my wife’s name. ( placed middle as double last name. She wanted her last name moved to middle and use my name last name). 

The error wasn’t caught until after National ID card, Drivers License, and social security were updated with incorrect name. 

 

Error corrected after 1 month. 

Her district office took and would not give us the “incorrect name change document “. 

We do now have a document showing her first and last name (not using my name) small village district headman wouldn’t issue with a middle name. 

 

*****Question. Should this incorrect name be disclosed on a IR1/CR1 visa application?

 

Also since district headman took the name change document we don’t have an original of the wrong name, only a copy. 

 

Thoughts on the issue appreciated.

The visa application requires ALL names she has ever used. 

 

1. Her name at birth

2. She changed her first name prior to us meeting.

according to the visa requirements i must list ALL names she ever used.

 

1, 2, and 4 are most certainly required 

 

must I include #3 also? It was incorrectly  issued with her last name and my last name as her last name (we intended her last name to become a middle name). Neither of us caught the error until a week later and we corrected error about 1 month after issued with removing my last name as her district headman refused to issue her an ID with a middle name (small village).   I have no original document to provide as headman at district took possession of that certificate showing the name change. I have only an electronic scan made of the original. 

 

I have her birth certificate to prove #1

i have a name change document issued for #2

i have a name change document issued for #4

She never used the double last name, so I wouldn't say she did.  It was an error.  You might want to explain it in a separate document, in the interest of full disclosure, but an administrative government error that was corrected, does not constitute her USE of the name.
