Medical History for Knightsbridge

Quick question that i hope someone will be able to help with.

 

Does anyone know how much medical history is required for your medical in Knightsbridge, London? The reason I ask is that my GP has told me I go back to 2007 on computer records which is no problem but they don't have the resources to add anything before that from my clerical records without charging me.

 

If clerical records are required, do they just need a summary?

06-04-2007 = TSC stamps postal return-receipt for I-129f.

06-11-2007 = NOA1 date (unknown to me).

07-20-2007 = Phoned Immigration Officer; got WAC#; where's NOA1?

09-25-2007 = Touch (first-ever).

09-28-2007 = NOA1, 23 days after their 45-day promise to send it (grrrr).

10-20 & 11-14-2007 = Phoned ImmOffs; "still pending."

12-11-2007 = 180 days; file is "between workstations, may be early Jan."; touches 12/11 & 12/12.

12-18-2007 = Call; file is with Division 9 ofcr. (bckgrnd check); e-prompt to shake it; touch.

12-19-2007 = NOA2 by e-mail & web, dated 12-18-07 (187 days; 201 per VJ); in mail 12/24/07.

01-09-2008 = File from USCIS to NVC, 1-4-08; NVC creates file, 1/15/08; to consulate 1/16/08.

01-23-2008 = Consulate gets file; outdated Packet 4 mailed to fiancee 1/27/08; rec'd 3/3/08.

04-29-2008 = Fiancee's 4-min. consular interview, 8:30 a.m.; much evidence brought but not allowed to be presented (consul: "More proof! Second interview! Bring your fiance!").

05-05-2008 = Infuriating $12 call to non-English-speaking consulate appointment-setter.

05-06-2008 = Better $12 call to English-speaker; "joint" interview date 6/30/08 (my selection).

06-30-2008 = Stokes Interrogations w/Ecuadorian (not USC); "wait 2 weeks; we'll mail her."

07-2008 = Daily calls to DOS: "currently processing"; 8/05 = Phoned consulate, got Section Chief; wrote him.

08-07-08 = E-mail from consulate, promising to issue visa "as soon as we get her passport" (on 8/12, per DHL).

08-27-08 = Phoned consulate (they "couldn't find" our file); visa DHL'd 8/28; in hand 9/1; through POE on 10/9 with NO hassles(!).

Hiya hopefully I can help, 

 

You just need to ask your GP for a Patient Summary Care Record - showing your basic personal information, last visits to the docs, any medication you are on and full list of immunisations. The receptionist should be able to print it out for you, I had my medical in December and took what my docs thought was a summary down with me - (it just was my jab history and basic info), but they said it wasn't the full info needed when I was there! 

 

It wasn't an issue though, I was able to complete my appointment and then email through the correct form once I had gone back to my docs. 

 

I did receive an email from Knightsbridge confirming my appointment with required docs, I took a pic and tried to upload for you, but it's too big so here is a copy and paste - NOTE, the links won't work because it's a copy paste from my email but will give you an idea :)

 

Copy and Pasted from my email from Knightsbridge: 

  1. REQUIRED DOCUMENTS:

 

You are required to bring the following for each person having a medical examination:

  1. Original Passport or UK Driving Licence (photo card not paper licence)

               We do NOT accept copies of ID

  1. 4 X Identical Passport style photos - UK or US size

(we accept UK or US sized photographs)

  1. ACRO Police certificate (or a copy) dated within the last 12 months for anyone 16 years of age & over (if your police certificate is more than one page long please advise Knightsbridge Doctors ahead of your medical as you may need additional time with the Doctor. If you fail to tell us ahead of time you may be asked to come back to go over this at an additional fee).
  1. Vaccination records - Please provide a vaccination record for the panel to look over on the day to advise if any further vaccines are needed.

Please bring Red Books, if available.

  1. A Letter/Email from the embassy/NVC to confirm your case number and visa category
  1.  Summary of medical history from GP (NOT full medical records) and reports covering any medical conditions, disabilities, and prior serious illnesses you have been treated for or anything ongoing. This is a new requirement and may not yet be added to the checklist online or the US EMBASSY LONDON website.

We will keep your medical on hold if you do not have it with you on the day of the appointment.

Ask your GP for a Patient Summary/Care Summary.

  1. If you wear glasses or contact lenses please bring them along to the appointment
  1. If you have any significant/ongoing/previous medical conditions please ensure that you bring a report from your GP/Specialist/Consultant regarding this; including, medication, prognosis, management etc.

* If your medical notes are very extensive (more than 10 pages) please inform Knightsbridge Doctors before you come for your medical. This required extra time with the doctor to go through your notes. If you do not inform us of this, you will be charged for an appointment slot for the doctor to read these notes.

Lucy what was the difference between the first document you took down & the one they wanted? I have my patient summary but there isn’t anything except jab history (as I haven’t had health issues for a while), all other sections are (rightly I think) blank. My alternative is a full history which I know they don’t want.

The one I took down had my basic data info, list of jabs since birth and that was it. I did email them in advance and queried it as it's the most confusing requirement - they emailed back and said what I had would be fine, but I guess it's down to the doc on the day! 

 

He explained the form needed had more information - last 3 appointments and any data. He did say in some cases the information is blank so you may already have the right one.

Hello everyone,
I am in the process for a K1 visa, I have my medical in London in 2 weeks. I have a few doubts that are stressing me out.


1. I am from Italy and have lived in the UK for almost 3 years now. I have all the documents necessary for my medical. I recently got from my GP the patient care summary too but I was wondering whether I need my medical records from Italy too, it's not specified anywhere! (It is specified instead for criminal records).

Question 1 on their questionnaire (https://uk.usembassy.gov/wp-content/uploads/sites/16/2017/05/USA-MEDICAL-QUESTIONNAIRE_IV.pdf) asks if you've ever been hospitalised and I had to put YES: I got into a car accident in Italy (been in hospital under observation only), had a head trauma with temporary amnesia in Italy and a kidney infection in the UK (due to episode of kidney stones). I explained all these 3 in an attachment to the questionnaire. Do I need to find and take with me all the translated documentation from Italy? (it would be a problem to get them in time..)


2. In case I find out only when I'm at the medical that they also need info from my Italian records can they simply access them themselves or I have to provide them (translated) afterwards and therefore causing delays in my visa process?

3. I put NO on the question that asks "Have you ever had any kidney or liver disease" as it was only a single episode, nothing chronic. Is that correct?

Hi Gabriele, sorry for replying so late but I am in a similar situation now and was wondering what did you end up doing with your medical records from Italy? I am planning on translating all my vaccine records but also had a kidney stone when I was 8 years old (my only time in hospital)  so I am unsure what to do. Hope everything went smooth with your application and thank you in advance 😊
