Hi, my wife had her Medical Exam in Haiti prior to her K1 Visa Interview. The sealed envelope was given upon entering the US. That's all we know about her DS 3025. That's all that's missing applying for the AOS. What should we do? Is there another form we should use? Thanks.

What if I don't have a DS-3025?

If you are positive that you got the needed shots, then actually mailing in a DS-3025 photocopy is not necessary. The original form is with your other medical results and was turned over to USCIS at POE. That's actually the one that counts because it remained in the "chain of custody" so you didn't have an opportunity to alter it. The photocopy isn't really an official copy and is more for a reminder that you got complete at your visa exam. Of course without having a copy, you don't have the opportunity to see if the rest of the form was completed correctly."

Right now, I'm trying to decide if I should send the Petition as is without DS 3025 or I 693, or try to submit it with I 693, which would mean delaying the Petition Mailing for about a week. I'm hearing and reading both cases, with most of it being to file with something instead of nothing. What is acceptable?

Thanks so much, that helps a lot. My Wife is very certain that the Doctor gave her all She needed, but I want to be safe not sorry. Thanks.

Hello just a heads up when my husband had his medical in haiti he did not get all the required shots because they didn’t have all the vaccines. He ended up having to get 1 or 2 of them here 

Our K1 Journey    I-129f

Service Center : Texas Service Center   Transferred? California Service Center on 8/11/14

Consulate : Port au Prince, Haiti             I-129F Sent : 4/14/2014

I-129F NOA1 : 4/24/14                            I-129F NOA2 : 9/10/14

NVC Received : 9/24/14                          NVC Left : 9/26/14

Consulate Received : 10/6/14 CEAC status changed to ready

Packet 3 Received : 10/27/14 packet received by petitioner in USA ( beneficiary never received packet 3)

Medical: 10/30/14 Dr. Buteau                  Medical picked up: 11/3/14

Packet 3 Sent : 11/10/13.. Had to schedule interview appointment and attach confirmation receipt to packet

Interview Date : 12/1/14                           Interview Result : Approved !

Visa Received : 12/10/14 picked up at Jacmel location

US Entry : 12/15/14 Fort Lauderdale, Florida

Apply for Social Security Card: 12/30/14 Connecticut

Marriage: 1/26/15

 

Adjustment of Status

CIS Office : Hartford                                  Filed : 3/18/15

NOA : 3/25/15                                            Biometrics : 4/15/15

Approved: 8/31/15                                     Received: 9/8/15

Approved: 10/23/18 -no interview
