Where is the Passport Specialist in new york

I would like to ask everyone regarding CRBA since my fiance cant come during the interview in Manila, my  fiance need to complete the Ds 5507 form notarized by a passport specialitsy, my problem is that we dont know where to go...any adress or agency . AState of New York only pls

It looks to me that he just needs to fill it out and have a notary sign it as well. Then he can send it you through the mail or by email. 

 

The USA is not like the Philippines where only certain people from different agencies can do things. All he needs to do is fill it out and have it notarized.

Is it okay that the notirization wll be in he bank instead in a passport specialist? Thanks

It doesn't matter as long as it's a notary.
