Vaccine Supplement for I-693

Hi,

 

My wife entered from Canada on a K1 Visa. We're in the process of putting stuff together for the AOS, but I'm not quite sure what to do with the I-693. She had a medical exam in Canada, so I would assume she only needs the vaccine supplementary information. She should be completely up to date on her vaccine.

 

I've heard that there are local doctors that can transcribe the information into the I-693, but I'm not sure where I get the record of her previous exam. She's really not quite sure what they told her in Canada, but they gave her a CD that I'm not supposed to open (and I'm not sure what's on it). Is it normal to expect the CD given to her at the time of entry would have the medical records to complete this step?

The CD is just the chest xray...you can open it..it's hers to keep and do with what she pleases...

Did she do all her vaccines at the medical? If so she doesn't need them transcribed... read the pinned topic on this board about it.
