AP/EAD RFE response question? notarization ? (split/merged posts)

I received RFEs for my I-131 and I-765 saying, "Service records and information provided on your I-765/I-131 application conflict regarding your full legal name. Please submit a sworn affidavit indicating your full legal name...."

Do you actually have to get it notarized or can you just submit a sworn and signed affidavit?

I also have just received an RFE on my full legal name for forms I-131 and I-765.

Do you have to get the sworn affidavit notorized? The RFE doesn't say.

I am not sure if the notarization is a requirement, but it certainly doesn't hurt. So better to be safe and get it notarized.
