DS-160, Fees, and Children Joining at Later Date

Ok, I'm the petitioner and received an email from the embassy in Jakarta for my Fiance and now she has gotten an email as well. We understand that the DS-160 needs to be completed, but need some clarification on fees and procedure.

Who's coming now: Fiance + 4 yr old daughter

Planned for later (5 months or so): Fiance's 16yr old daughter and 14yr old son

Issue 1: Do we complete a separate DS-160 for everyone? That would mean 4 separate DS-160's.

Issue 2: If the two older children are coming later, do we delay paying the Visa fee's for them? Do we delay their DS-160, if separate?

Issue 3: Can we just use one DS-160 and pay fees for the 4 Visas?

DS-160 for the mom and the 4-year-old child.

they need medical too (pay for each of them)

and visa fee $265 for both of them (2 x $265)

then when the older kids are ready to come you will a bit before of course contact the embassy and ask for 2 K-2 follow to join and they

will tell you what to do. They will also need the above but not until they can come.
