confused about which affidavit of support form to use

I plan to co sponsor my cousins fiancée. My cousin doesn't earn enough to meet the poverty guidelines.

I don't know which affidavit of support form to use. Also, my tax return is a joint return, since I'm married and me and my wife file a joint return. Does my wife also need to fill out and sign an affidavit of support too? Any help would be appreciated.

12/22/07 -Application received in Chicago

12/29/07 -I-797C received via postal mail

01/10/08 - Received Biometrics appointmert notice

01/14/08 - Received email case transfered to CSC

01/18/08 - Biometrics appointment 8:00 am Louisville Kentucky

01/24/08 - I-485 Touched

01/25/08 - I-485 Touched

02/12/08 - EAD card production ordered

02/19/08 - EAD card received

03/26/08 - I-485 Touched

03/27/08 - I-485 Touched




01/26/10 - Sent I-751 to Vermont

01/29/10 - Application received in Vermont

02/03/10 - Check cleared bank

02/08/10 - Received NOA 1 (Dated 2/01/2010)

02/18/10 - Received biometrics notice

03/02/10 - Biometrics appointment

03/02/10 - Touch

03/03/10 - Touch

04/25/10 - I-765 Touched on a Sunday!!

06/04/10 - APPROVED!!

06/04/10 - I-551 Stamp in Passport

06/17/10 - Received "GREEN CARDS"

if you need the form to mail to the fiancée for her interview, the embassy want the I-134.

if it is an AOS after marriage it is going to be the I-864.

concerning the joint tax .. no idea .. maybe these links can help or someone has been through this before can help !

09/19/2016 -- Finally the embassy open interview date after long wait ... 35 Days (interview scheduled for Oct 10)

10/05/2016 -- Medical done. but no sealed envelope given.

10/10/2016 -- Interview day 1 ( pay the fee, deliver paperwork, finger printed ... and told to comeback tomorrow to be interviewed by a CO )

04/20/2017 -- EAD (only) Approval text & email notification (day 96 since case filled, 93 since received, 86 since NOA1)

04/24/2017 -- EAD & AP NOA2 Approval notice received in mail

04/25/2017 -- EAD (only) text & email notification that card was mailed

04/28/2017 -- EAD /AP combo card in hand

09/01/2017 -- AOS Approval text & email notification (day 230 since case filled, 227 since received, 220 since NOA1) Approved in 7.5 Month, Interview waived
