HELLO AGAIN,

Firstly, thank you all for the support.

I do have a question regarding the FORM I-693, If I took the medical exam two months ago in Nicaragua and brought back the xray cd and also a paper of my injections do I still need to fill in this form, take additional medical exams or what exactly now that I am about to adjust status and need to fill in these forms.

You don't HAVE to according to the instructions on the form, especially since you did it not too long ago! However if you don't have a complete series of vaccinations you MIGHT get an RFE for that. I guess it depends if the immigrations dude looking at your form is having a good day or is up to speed with the requirements. The doc here only charged 50$ to transfer the info to the form I-693 so we went ahead and did it. Didn't have a prob. People who send the DS form also don't have a prob.

You have a copy of the DS-20whatever form right? The doc who did it in Nic should have given you one

So it's pretty much up to you to send what you have.
