Form 751 : General questions

I have following general questions about the form:

1. How to get ELIS account number?

(Currently ELIS website says that "If you paid the USCIS Immigrant Fee before August 31, 2015, you will no longer have access to your online account."

We had created ELIS account to pay the fees for Green Card. It was created before Aug, 31, 2015. So we are not able to login)

2. We already filled "SSN" and "A number" in Part 1 question 7 and 8, Do we really need to fill ELIS number?

3. Last 3 parts (Part 9, 10, 11) doesn't apply to us. We have kept them blank/not filled. Do we still need to send them as blank pages OR we just don't sent those pages

- Part 9 (interpreter)

- Part 10 (Attorney)

- Part 11 (Additional Information)

4. Do we need to fill NA in any of the not applicable fields? For example we have only 1 kid and in the form they have space to enter details of 5 kids. Do we need to enter NA or we can keep blank.

5. Sign and Date on the form should be within 90 day expiry of the GC? OR 90 day rule only applies to sending the post.

I say for the ELIS number call the customer service and ask them for help. ( USCIS National Customer Service Center at 1-800-375-5283 or 1-800-767-1833)

Also, include all the pages on the form, yes, even the blank ones. It's better you send them in, than have them return your application because you didn't fill the from correctly.
