Following a large amount of messages I've received about this i wanted to make a post. Knightsbridge have very recently changed the wording on the ds3025 form as a lot of people were complaining about the immigration box being ticked. Mine says 'this k nonimmigrant visa applicant did receive the required vaccines and th e us vaccination requirements are complete for this applicant.

Now i can't say for past applicants but that is what they are putting on there now. So hopefully this can help ease a few over stressed minds.

So they handed out page 2 to you with the additional note. Maybe they have been reading VJ and all the times I have said the nurse at Knightsbridge is clueless about AOS procedures and how their actions cost K1s time and money because they won't tick the box.

It is a positive step to get the added note.

That's great I suspect Knightsbridge was tired of having angry customers. Now if only the people handling AOS would pay attention to that.

Just an add on, I contacted Knightsbridge to ask why I didn't have a page two from my medical in February, they kindly emailed me page one and two and apologised.

I intend to print the email and both pages for AOS. They were really helpful and got back to me really quick.

Typically page two is never included unless some note has to be added for why you couldn't be vaccinated...like no MMR because you were pregnant. You wouldn't need a blank page two because the doctor signature and all info is on page one. But if they added the remark like AMZJ got, then they would give page two as well.

And was your page two blank of any additional notations? If so, then you don't need it. It offers nothing to USCIS. If you had a contradiction waiver explained on it, they would have given it to you initially. If they added a remark (like on the OP's) after the fact, then it's not on the original found in your medical report and used by USCIS.

Notice page two is only if you had an allergy to immunizations, pregnancy, etc.

Mine is like AMZJs, it's stated that I've had all the required vaccinations required for the K1 and is signed and dated.
