How to include part time contractor work?

Hi all,

I am in the process of finalizing my N400 package and have a question on how to include contractor work in employment history. I already have a full time job but started a part time contractor work last month. I work from home couple hours a week, getting per diem payments, and will be done with it within a few months. Should I include this work as self employment and put my home address as my work address? Or include the company I do the work as employer? Please advice if you have other suggestions.

I guess no one in this forum had a similar situation. No answers :( Anyway after googling little bit I found a similar situation at http://forums.immigration.com/threads/can-i-skip-some-work-related-history-in-n-400-need-expert-advise-please-help.308078/

I decided to go with their suggestion include the contractor work as self-employment with my home address, and occupation part time independent consultant.

If you are a contractor and receive 1099 income from the company you're doing the work for, you consider this self-employment and should list yourself as the "employer". That's what I did.
