Hello all, interview is coming up and I have a few questions.

The interview is in Japan by the way, not sure how relevant that'll be if at all.

Anyway, I noticed the I-134 says it expires 2/29/16. Are they going to update the file soon? Should I be ok with just using the one on there now?

Also, for the bank statement document I do online banking. I was able to download a document with the Capital One logo on it, date, my name and address.

Account Number

Account Open Date
Total Deposits for Last 12 Months
3 Month Average Balance
3 Month Lowest Balance
Available Balance a sof (date that document was generated)

It just isn't signed or anything. Even though it isn't signed by anyone should I still be good with this?

Lastly, a couple questions on the current I-134

On number 10 do I put just my fiance? Do I put her two children on there as well?

On number 11 it says I intend/do not intend to make specific contributions to support the persons in item 3. It says if you check I intend you have to explain it... I find it a little hard to answer haha. If anyone has any clarification for me that'd be great.

I just went to bank and showed them the I-134 instruction's for bank letter. It's just a one paragraph letter RE: account verification and balance. Just a simple statement from bank officer on bank letterhead with signature and contact info. Showing date account opened, account number, total deposits for last year, current balance, and bank officer's contact phone and email.

It only took bank officer about 15 minutes to get this letter ready for me:

The OMB(Office of Management and Budget) expiration date only matters internally for the USCIS. That expiration date has nothing to do with whether or not it will be acceptable for you to use. The edition date is what matters. The accepted edition dates for the forms are listed on the site where the forms can be found and downloaded.

The I-134 states this. > https://www.uscis.gov/i-134

Edition Date

02/19/14. A new edition of this form is coming soon. In the meantime, customers may file using the 02/19/14 edition. We will also accept previous editions.

If your income meets the requirement, then no need for bank statements or bank letter. Just provide complete copy of most recent taxes and proof of your current income (letter from employer and/or recent pay stubs).

Yeah, unfortunately I just don't have an actual physical bank here where I live. I'll just include the document I can download from the website though. Maybe give them a call and see if they can send me something real quick or something.
