Over a year since AOS I-797 NOA1 received. What now? (corrected title)

Timeline

My wife and I filed her I-797 in February of 2015. They asked her to have another physical done in December of 2015 since her medical data was over a year old, but that has been all we have heard since we filed last year. We have several friends who have already received their LPR status and they filed after we did. Checking online gives us the same message we have seen since April of last year. Do they usually take this long?

Also, her employment authorization is almost expired. Will they send a renewal or do we need to do that ourselves?

Do you mean the I-485 for Adjustment of Status? The I-797 is a notice form. Did you get the medical completed by a civil surgeon then send the, sealed, I-693 to USCIS with the original RFE document?
