I am assembling the I-485 package. And following the checklist on VJ site.

Do I include the I-765 application for employment authorization with the package?

As certain as I can be based on USCIS' own instructions and having myself sent them $1070 in December. I sent a single check to cover the filing and biometric fees and it was accepted.

$1070 is the correct for most adults. You should also consider including the I-131 Advance Parole (AP) app, with the EAD. It is an easy application for fill out, follow the VJ example forms, http://www.visajourney.com/examples/INS-Form-I-131.pdf . As mentioned in the previously provided link, there is no additional charge for concurrently filing the AOS/EAD/AP apps. together. Additionally, there have been several postings where the folks have filed their AOS, then realize that they need to get back to their home country quickly for some emergency (family, etc.) and then have to scramble to get an emergency AP.

You don't have to pay the normal $380 filing fee for the I-765 because you are filing it along with the I-485. I supposed you could consider it "included" with the I-485 but, rather, it's being waived.
