Information about parents on N400

I was adopted in the US but still keep in contact with my biological mother. Should I leave the information about my mother blank or put down my adoptive mother who is a US citizen?

If the adoption procedures had been completed through the court, you have nothing to do with your biological parents. From the date the judge signed the court order, everything about your "parents" will be about the adoptive parents.

No. because they have no formal/ official ties to you anymore, since you are legally adopted.

legally adopted means your biological parents gave up their rights, they are no longer considered your parents, so your adoptive parents are your parents
