Hi all (F) !

Hope everyone is having a good day.. or night!

I have called US Embassy yesterday and have scheduled my interview on Dec. 7th. My fiancee and I were hoping to schedule it within November but the embassy said they don't have any open slot for this month anymore.

I have already filled out the DS-160 but havent clicked "SIGN" yet because there are some questions that I'm unsure of what answer to put in there. I have called the US Embassy and asked the customer service for help but she declined..

So I am here again.. I need your help to make me understand.. pretty please. I just want to make sure that I am not just putting unnecessary answers to questions that I don't even need to answer.

1. Marital Status - my previous marriage was already annulled but the drop down from that question doesn't have the word "Annulled" in the list. Will it be safe to click on "DIVORCED" or "OTHERS" instead?

2. National Identification Number - should I just put my Tax Identification Number here?

3. Passport Book Number - I'm lost. I know that this is different from the passport number.

4. US Point of contact info - Fiancee? My mom told me we have relatives living in the states but she doesn't even know their names (lol) so maybe it is safe to click "no"?

5. Vaccinations - should I just click Yes and just refer to St. Luke's documentations after my medical?

6. Other names used - I dont have any aliases but how about my married name? I used my ex husband's family name when my marriage was not yet annulled.

I am kinda confused (yet, again).. maybe I am just nervous and I am scared that my application will be denied just because I did not understand and I made a mistake. I put everything to the best of my knowledge in that form and I just want to make sure that I understood every questions before I click "Sign".

2. National Identification Number - should I just put my Tax Identification Number here? That number will work.

3. Passport Book Number - I'm lost. I know that this is different from the passport number. Use your passport number

4. US Point of contact info - Fiancee? My mom told me we have relatives living in the states but she doesn't even know their names (lol) so maybe it is safe to click "no"? Use the contact information where you will have your domicile - Fiancée is fine.

5. Vaccinations - should I just click Yes and just refer to St. Luke's documentations after my medical? Use YES as your answer as you will have had all your inoculations once you arrive for your interview.

6. Other names used - I dont have any aliases but how about my married name? I used my ex husband's family name when my marriage was not yet annulled. Previous married name is considered "other name" and should be listed whenever asked. Be sure to get your NBI in all names. It's referred to as AKA names.

I am kinda confused (yet, again).. maybe I am just nervous and I am scared that my application will be denied just because I did not understand and I made a mistake. 2. National Identification Number - should I just put my Tax Identification Number here? - Yes, i used TIN or SSS will also do

3. Passport Book Number - I'm lost. I know that this is different from the passport number. NOT APPLICABLE, Philippines passports has no book no.

4. US Point of contact info - Fiancee? My mom told me we have relatives living in the states but she doesn't even know their names (lol) so maybe it is safe to click "no"? - Fiance

My NBI clearance only shows my maiden name. I guess I need to apply for another clearance using my married name again.

You need an NBI clearance with an AKA - your previous married name.

PS. I also had an annulment and declared that name on my DS 160 and NBI.

Thanks, Cristina. I have applied a new nbi clearance online and i am scheduled to go to their office tomorrow morning. I hope everything will turn out fine. I know I need to relax but this whole process is making me nervous and cant wait until this journey will end in a positive note.

Yes status should be single. I always use single after my annulment. That what annulment means, even on my passport renewal, I used Single.

Thanks again! Went to NBI yesterday, and theyre offline. Waited for 3 hours only to tell us to go to their satellite office Robinsons Manila (only those who applied online and paid at banks/bayad center). After waiting for an hour I finally had my pics and fingerprints taken.

Medical will be on Nov. 23rd. And if the medical turns out ok, we will be going to do the Seminar. Interview will be on Dec. 7. Praying hard that everything will be ok.

Hi guys! I just received the packet from the US Embassy today. They gave me a copy of the I-129F and there I did not put my previous name/alias. Is it safe if I put my previous name in the DS-160? Now, I am worried.

I-129 asked about other names used (10) and Name of Prior spouse/s (13). You did not declare any of this on your I-129F?
