I (the beneficiary) am a citizen of two countries, so I have two different passports that I've used to enter the US on different occasions and with different visas.

Passport A is from my country of birth, has the last name I've used all my life, and had my F1 visa on it (I was a student in the US until last year), and also has a B2 visa.

Passport B was obtained by descent, has a slightly different (by two letters) version of my last name, and is the one I've been using this past year to visit my fiancée in the US (with ESTA).

My fiancée and I are preparing to submit the I-129F packet, and I most likely will be using passport A for this process.

Since we're going to use the stamps on passport B/boarding tickets as evidence of me visiting, should we list my name as it appears on passport B in the "other names used" section of the I-129F? Should we also have some sort of disclaimer on the cover letter?

Also, if I instead decide to go with passport B for this process, what should I do about my last name? My SSN and tax information, among other important documents, use the name on passport A...

Any advice?

Absolutely you should write down any and ALL names you have used. I"m not sure why you've never made sure both passports were the same, but you must list all your legal names.

And on the name issue, since passport B was passed down by descent the country required that it has the last name of my ancestors, which was altered when my family came to the country where I was born and basically became the new family name. It's frustrating but there was nothing I could do about it.

I would suggest the Country of Birth passport, as that would be the matching name to the birth certificate. I would also list under other names used the second passport. Include copies of both with an explanation regarding the differences.
