Adjustment of Status filing question

I thought I saw on here awhile ago a sample of a cover letter for adjustment of status. Now I can't find it. Does anyone have a cover letter than I can see? I am not really sure where to start on making one and we have everything else ready to go.

j. One passport-type color photo of the intending immigrant

k. Three color family photos along with a third party letter of support to establish evidence of an ongoing marital union between the petitioner and the beneficiary

2. I-485 Package

b. $395 check for Form I-485 and biometric services payable to the Department of Homeland Security

c. Form I-485: Application to Register Permanent Residence or Adjust Status

d. Copies of the intending immigrant’s passports

i. Biographical page of current passport

ii. Biographical page of invalidated passport

iii. Copy of F1 visa

iv. Copy of entry stamp

v. Copy of I-94 (front and back)

e. Copy of EAD, I-20 and Certificate of Completion issued by City College San Francisco as evidence of the intending immigrant’s F1 Student Status

f. Copy of the intending immigrant’s international birth certificate

g. Certified copy of marriage certificate

h. Two passport-type color photos of the intending immigrant

i. Form G-325A filled out by the intending immigrant, signed and dated

j. Form I-693 for intending immigrant seeking adjustment of status (in sealed envelope)

k. Form I-864 Affidavit of Support

i. Copies of Federal individual income tax returns for 2006, 2005 and 2004 along with every Form W-2 and Form 1099 that relates to the submitted returns

ii. Pay stubs from the last six months

iii. Employer letter
