Mailing Address?

My fiancee and I have finished his Adjustment of Status and EAD paperwork, and I'm very confused as to where we send the EAD paperwork. We are filing them at the same time, so would our category be ©(9) or (a)(6)? Are they both to be sent to the Chicago address? We live in Ohio and our I-129F was processed at CSC, if that helps. I've seen conflicting information as to where it's mailed to elsewhere on the internet.
