How do I answer the surnames part in the ds-260 form? Do I put in the middle name and the last name or just the last name? Thanks

In the surnames part you add just your last names. The middle name goes in the ''given names'' part along with your first name. This must match the names listed in your travel document.

thank you for replying.

So if my name is George Michael J. Peterson, in my given name I put George Michael. And in the surnames I put James (middle initial) Peterson. Is that right? Or just Peterson in the surnames and George Michael in the given name
