DS-3025 US Vaccination requirements COMPLETE" is not ticked...

Bit puzzled here - hope you folks can help with this...

So...done all the Knightsbridge stuff a couple of weeks ago, and I got my DS-3025 - all well and good :)

Lots of A's and B's and other bits on the tick list.

...and down the bottom, though it's signed and dated the box marked US Vaccination requirements COMPLETE" is not ticked...

So..I wrote to the doctors and they said:


I just have a question for you before I can send you a new form; what type of visa are you applying for?

If it is a K1 Visa then the ‘completed’ box does not need to be ticked.

I look forward to finding out from you what type of visa it is so that we will be able to send you an amended form if we need to. "

and I think they are wrong - I'm a K1 visa, and this lack of a tick is going to give me a $600 surgeon certification issue if I want to use it for the AOS

Any thoughts please?

As long as you have the required vaccinations, you can submit a copy of the DS-3025 with your AOS application. No need to see a civil surgeon.

09/05/2018 -- Approved without interview!

09/13/2018 -- 10 Yr Green card Received

What if I don't have a DS-3025?

"If you are positive that you got the needed shots, then actually mailing in a DS-3025 photocopy is not necessary. The original form is with your other medical results and was turned over to USCIS at POE. That's actually the one that counts because it remained in the "chain of custody" so you didn't have an opportunity to alter it. The photocopy isn't really an official copy and is more for a reminder that you got complete at your visa exam. Of course without having a copy, you don't have the opportunity to see if the rest of the form was completed correctly."

OK - so from elsewhere in that thread it appears that it not being ticked is not a problem - plus - it's in the system so it can't be changed - ah well :)

....update...and confirmed by Knightsbridge Doctors...

Due to the fact that it is a K1 visa, we are in fact unable to tick the ‘completed’ box. This is because the K1 visa is currently NOT an immigrant visa. Therefore under US Immigration regulations we are unable to tick that box for this type of visa.

hey ho - so have to strike that bit in the advice about people being able to ask for it to be ticked...not going to happen :)

Hi Lauren and Kevin,

Did you have any boxes ticked in the Results section? We are filling out our forms now and our DS-3025 has nothing marked under results... just want to make sure that's normal!

06-11-2007 = NOA1 date (unknown to me).

07-20-2007 = Phoned Immigration Officer; got WAC#; where's NOA1?

09-25-2007 = Touch (first-ever).

09-28-2007 = NOA1, 23 days after their 45-day promise to send it (grrrr).

10-20 & 11-14-2007 = Phoned ImmOffs; "still pending."

12-11-2007 = 180 days; file is "between workstations, may be early Jan."; touches 12/11 & 12/12.

12-18-2007 = Call; file is with Division 9 ofcr. (bckgrnd check); e-prompt to shake it; touch.

12-19-2007 = NOA2 by e-mail & web, dated 12-18-07 (187 days; 201 per VJ); in mail 12/24/07.

01-09-2008 = File from USCIS to NVC, 1-4-08; NVC creates file, 1/15/08; to consulate 1/16/08.

01-23-2008 = Consulate gets file; outdated Packet 4 mailed to fiancee 1/27/08; rec'd 3/3/08.

04-29-2008 = Fiancee's 4-min. consular interview, 8:30 a.m.; much evidence brought but not allowed to be presented (consul: "More proof! Second interview! Bring your fiance!").

05-05-2008 = Infuriating $12 call to non-English-speaking consulate appointment-setter.

05-06-2008 = Better $12 call to English-speaker; "joint" interview date 6/30/08 (my selection).

06-30-2008 = Stokes Interrogations w/Ecuadorian (not USC); "wait 2 weeks; we'll mail her."

07-2008 = Daily calls to DOS: "currently processing"; 8/05 = Phoned consulate, got Section Chief; wrote him.

08-07-08 = E-mail from consulate, promising to issue visa "as soon as we get her passport" (on 8/12, per DHL).

08-27-08 = Phoned consulate (they "couldn't find" our file); visa DHL'd 8/28; in hand 9/1; through POE on 10/9 with NO hassles(!).

If by the results column you mean the first column with the tickboxes?

Yes, I had several ticked with dates where I had given details of vaccines received with dates to the nurse.

But I have a horrid feeling we'll have to get the Civil surgeon dude to do us extra stuff, as it does not look finished to me.
