Doing my AOS tonight for my wife. A quick response would be appreciated.

As I understand it, I don't have to submit any Medical exam or vaccination paperwork because my spouse did all that within the year at her country and submitted the sealed brown envelope upon arrival to the USA. (Nothing wrong with her vaccinations)

So no need to submit this portion of the AOS right?

Just found out my NOA2 approved Oct 25, 2013 is still good. Was automatically extended by the embassy until one year of the last date of notification which is Dec 3, 2013. It's a race now.

Near the bottom of the DS-3025 there are two boxes one is if all US vaccination required are met and the other is vaccinations are still required. That is how you will know if you need to see a civil surgeon or not. If vaccinations are still required then you need to make an appointment with a CS. Shop around the prices are all over the board.

We did not go to a CS but submitted the DS-3025 along with these instructions: (Green Card in hand with no RFE!)

If you already had a medical exam overseas or If you were admitted as a:

a. K-1 fiancé(e) or a K-2 child of a K-1 fiancé(e); or as a

b. K-3 spouse of a U.S. citizen or a K-4 child of a K-3 spouse of a U.S. citizen; and

c. You received a medical examination prior to admission, then:

You are not required to have another medical examination as long as your Form I-485 is filed within one year of your overseas medical examination, and the medical examination did not reveal a Class A medical condition, or if you did have a Class A medical condition, you received a waiver of inadmissibility and have complied with the terms and conditions on the waiver.

Even if a new medical examination is not required, you must still show proof that you complied with the vaccination requirements. If the vaccination record (DS-3025) was not properly completed and included as part of the original overseas medical examination report, you will have to have the vaccination report completed by a designated civil surgeon. In this case, you are required to submit the vaccination record and page 1 of Form I-693.

You do not need the medical exam tests repeated if your medical was done within a year of filing for AOS (SLEC Philippines meets these qualification), that is all you need. No full, part of an exam is necessary once you arrive in the USA nor any tests. Your vaccinations need to be complete. Attach the following pages to your I-495 along with your DS-3025.

You need to submit a copy of the DS-3025 with the I-485, and I suggest you make a copy of page 5 of the I-485 instructions and highly section 7c then attach it to the DS-3025... many idiots at USCIS that don't know the process.
