I work as an independent distributor for an internet product brokerage company, I just started there and haven't gotten payed yet but will in the future. So I have two questions

1. Do I list it as employment history even though I haven't been payed yet?

2. What do I put down as an address? I do my work from home on the internet, but the main company has its headquarters in Greensboro North Carolina, but I don't do any work there and have never even been there.

If you have already started the job when you fill out the form, then list it, whether paid yet or not. If you are a self-employed independent contractor, then you can simply put Self-Employed, no address needed, and then list your occupation.
