If you have a completed vaccine worksheet, you don't have I-693 transcribed.

If your DS-3025 (vaccine worksheet) is uncompleted and/or uncorrected, you will have to have it transcribed to I-693 form, which costs variably from 50 - 100. It is fast in-and-out visit.

If your medical report (which you took in your country) is more than 1 year old, you need to do complete I-693 again. Need 2-3 visits. Costs $200 and up.
